Accident Circumstances

 

After the take-off from queretaro Airport the aircraft lost power and the pilot decided to carry out an emergency landing at an unprepared crops field; irregular terrain with many stones.

 

Aircraft Damage

 

The aircraft sustained the following damage: left main and nose landing gear were severed, both wings’ leading edges and spars are damaged, the nose section was severed, the propeller blades are bent, and the engine requires a sudden-stop inspection.

Accident Circumstances

 

On 29th January 2016, while providing top-cover protection for another Bell 206 helicopter during a defoliating-spray operation to destroy poppy plantations, 42 nautical miles southwest of Ciudad Altamirano, Guerrero, the pilot reported that he had heard an unusual noise coming from the engine; so, he decided to land beside a river to check the helicopter condition. But, in his hurry to land, the pilot touched down on uneven rocks and his subsequent over-controlling caused a main rotor blade to hit the upper wire cutter

 

Damage

Due to the heavy landing, the helicopter sustained damage to: the main rotor hub assembly (including the pitch links); the swashplate assembly; one main rotor blade; the upper wire cutter. In addition, the engine and transmission require sudden stop inspections.

 

As a result of the rescue operation the two lower windshields broke, and the belly shell structures have multiple scratches and gouges in the honeycomb structure.

Salvage Details

The Mi-17 V5 helicopter, registration ANX-2224, serial number 484M02, has a total time since new of 1,346.0 flight hours, and 3,718 total landings. It is equipped with two forward cabin doors and aft hydraulic operated platform.

 

Accident Circumstances

While the helicopter was carrying out armed surveillance in a restricted area at the north-west of the Sea of Cortez (to protect species in high risk of extinction because of poaching), the Mi-17 V5 helicopter, with 3 crewmen and 9 passengers (all marines) on board, fell into the sea for unknown reasons..

The helicopter was recovered after sunk in slat water for almost 4 days.

 

Damage

The nose section crushed and distorted due to the impact against the sea surface, the forward cabin bulkhead distorted at the right and lower sides (just aft of the nose landing gear leg), the right external fuel tank crushed, the main rotor hub seemed complete, with the five main rotor pitch links in position, the tail boom with all drive shafting, tail rotor, vertical and horizontal stabilizers was broken and separated (the diagonal cut on the tail boom seems that was caused by the impact of one or two of the main rotor blades), two of the five main rotor blades were broken and separated, and were not recovered, their attachments to the hub remained. The three recovered main rotor blades were broken at the middle and tip sections, but two of the main spar nitrogen indicators showed them to be unbroken, the internal 200-litre fuel tank was crushed, the three tail rotor blades had no major damage. oxidation all over the cockpit and cabin structures due to salt water contamination.

Accident Circumstances

 

On 3rd October 2019, while providing top-cover helicopter, during a defoliating-spray operation to destroy poppy plantations, the crew of the Bell 206 L3, noticed installations for drying marijuana in the clearing in the forest. The pilot decided to descend to make a detailed analysis to inform his superior officers. However, when approaching the clearing, the tail rotor struck a tree and detached; the aircraft then spun to the ground and ended wedged between two trees.

 

Damage

The helicopter sustained the following major damage: Deformation and distortion throughout the airframe, all dynamic components are considered scrap by the Manufacturer, the windshields and cockpit windows are scrap, the main rotor blades broke, the undercarriage distorted and detached, the tail boom section, including the tail rotor drive shaft, broke, the tail rotor broke and detached and the rotating components of the engine are considered scrap by the Manufacturer.

Accident Circumstances

 

On 19th January 2020, while descending to land at the airstrip of the Army Operations Base #1 in Guadalupe y Calvo, Chihuahua, the helicopter descended rapidly, hit the ground with the left landing skid first at a very high rate of descent, the skid broke and rolled over

On 22nd May 2018, while landing at Toncontín Airport at Tegucigalpa, Honduras, from Austin, Texas, the Gulfstream

the steep slope beyond, collided with a light post and damaged 100 meters of 3 high-tension cables of the electrical power
network.

Accident Circumstances

On 4th August 2020, while the aircraft was landing after completion of a coastal patrol mission, the two-pilot crew reported strong crosswind from the right near the runway threshold. Although the pilot applied full left rudder after touch down, the tail-wheel aircraft (with centre of gravity behind the main wheels) yawed like a weathercock into wind and then the turn tightened. It stopped on the right side of the runway heading backwards.

Damage

The tubular structure of the lower right fuselage is broken at the attachments of the landing gear leg and the damper. The aft landing gear leg attachment bolt is broken. Right step is broken at its attachment to the fuselage. Upper attachment of the right main landing gear damper broken. Left main landing gear attachment lug has a hairline crack on the soldered union. Upper right engine attachment support structure has a hairline crack. Upper aft fuselage structure where the empennage assembly attaches is slightly bent. Lower aft fuselage structure where the tail skid attaches is also slightly bent. Right wing tip metallic reinforcement is bent. Right elevator structure is slightly bent. Right wing aileron structure (outer rib) is bent.

On 10th June 2021, during a training session of landing in confined areas, 15 nautical miles northeast of Military Air Base #1, the pilot-in-command was attempting to land in a clearing surrounded by trees and prickly pear cactus. However, during his low approach, the helicopter hit a tree with the main rotor blades and then landed heavily.

Aircraft Details

Accident Circumstances

On 15th October 2020, during a training session, the instructor pilot reduced engine power after take-off to simulate an engine stoppage. The pilot-in-training attempted to land but brought the cyclic stick backwards at touchdown and the main rotor blades came down and chopped off the tail rotor assembly and fin. The helicopter landed very heavy with low rotor RPM.

Damage

The vertical fin, transmission and all the tail section (tail boom, tail rotor gearbox, tail rotor blades, tail rotor hub assembly and tail rotor shafts) are scrap. The undercarriage is splayed, so the cross tubes need replacing. The engine requires hard landing and sudden stop inspections. All the avionics need to be bench-tested.

Aircraft Details

The Airbus Helicopters MBB BK117-C2, XA-UQV, serial number 9497, has logged 5,835 total flight hours since new. The two Turbomeca Arriel 1E2 engines, serial numbers 47377 (left), and 47374 (right), have a total time since new of 6,778 and 6,956 flight hours respectively.

Accident Circumstances

On 12th March 2021, the helicopter took-off from an oil rig in the Gulf of Mexico bound for the helipad of the harbour of Dos Bocas, Tabasco. According to the pilot’s report, during his final approach to land, the helicopter suddenly started to yaw (turn around its vertical axis) to the right: the tail rotor blade pitch (controlled by pedals in the cockpit) had reached its limit, so the helicopter started turning clockwise. The main rotor blades turn anti-clockwise, so the fuselage will turn clockwise if there is not enough, or no, counter-thrust from the tail rotor. Therefore, the pilot lowered the collective lever (pitch control) and decelerated the engines to attempt a normal landing, but hit the ground exceptionally heavily while still yawing.

Damage

Extensive damage to the fuselage structure. Skid landing gear collapsed under 10G+ deceleration of heavy landing. Compression of cabin  at cabin floor and transmission deck level structure visible.  Front vertical beam buckled and cabin twisted right and downwards.

The airframe is in Mielec, Poland, the engines in Canada (at Pratt & Whitney) and the rest of the aircraft at Leonardo Helicopters Philadelphia. The complete helicopter will be gathered and delivered in Philadelphia.

Accident Circumstances

On 25th September2019, during a training session at Military Air Base #1, Santa Lucia, State of Mexico, the helicopter landed heavily on the right landing gear first and collapsed, the landing gear sponson structure was badly distorted.

Damage

The impact on the landing gear support box caused damage on the roof, floor and vertical panels that linked the two; therefore, the whole central section of the cabin should be replaced. The engines need a light overhaul; the landing gear is scrap. The tail rotor gearbox, main rotor dampers (4) and tail rotor servo-actuator were overhauled by Leonardo. Also, Leonardo repaired the main rotor blades (4), rotor blades (2) and main rotor servo-actuators (3).
